For referring practitioners

Two ways to refer a patient

You can refer a patient to us by downloading and completing our referral pad, or by submitting the online form below. Either reaches the same place and is picked up by our treatment coordinators on the next business day.

If you would rather talk a case through before referring, call the practice and ask for the orthodontist you would like an opinion from. Growth and timing questions are welcome, including whether a patient is better seen now or reviewed in twelve months.

Email an OPG, lateral ceph, clinical photographs or a letter to reception@midlandorthodontists.com.au with the patient’s name, and our coordinators will attach them to this referral.

Referrals are handled by our treatment coordinators. For an urgent case, call 08 9274 6861.
